A chronically ill enrollee is defined as an MA member with one or more complex chronic conditions, who is at risk for hospitalization or other adverse health outcomes, and who requires intensive care coordination. This proposal would shift the burden from CMS to the MA organization to demonstrate compliance with this standard.
