QliqSOFT: Addressing the Challenge of Collecting SDOH Information

Collecting social determinants of health (SDOH) information from patients is challenging. It is not easy to ask if a person is experience food insecurity or is currently unhoused. QliqSOFT is making it easier to get this important information while maintaining dignity.

Healthcare IT Today caught up with Bobbi Weber, VP of Product Marketing & Customer Success at QliqSOFT, a company that provides HIPAA-compliant, real-time communication between doctors, nurses, caregivers and patients. We wanted to find out more about how the company is helping healthcare organization collect SDOH information.

SDOH Data is Vital for ACO Reach

CMS’ Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) model is designed to improve the quality of care for people with traditional Medicare. One of the main goals of this model is to “Advance health equity to bring the benefits of accountable care to underserved communities.”

“Collecting the SDOH data necessary for ACO REACH can be awkward and challenging,” said Weber. “What we are finding is that patients are much more comfortable providing the information through a chatbot rather than in a conversation with a clinician.”

QliqSOFT sends the questions to the patient as a link to a chatbot which asks the needed questions via a mobile or online interface. The questions that are asked and how they are asked are completely configurable by healthcare organizations.
